INSTRUCTIONS
for “Request for Research Associateship Program”

The information requested on the referenced form is instrumental for National Academies consideration of a laboratory's request to initiate or continue a Research Associateship program. It will be used by the Associateship Programs' Office and will be made available to National Academies site visitors and to members of the Commission on Human Resources. Please provide information that is complete, yet concise, keeping in mind the objectives of the National Academies Research Associateship Programs. Supplementary sheets should be numbered sequentially to correspond to item numbers on the Request form.

Item No. 4 Provide complete name, title, address, phone number, fax number and email address of the principal administrative officer (i.e., Director, Chief Scientist, etc.)

Item No. 5 The National Academies Program Administrators prefer to work in close collaboration with staff members who are closely affiliated with research management of the laboratory. Provide complete name, title, address, phone number, fax number and email address of the person who will serve as the laboratory program representative.

Item No. 6 Provide complete name, title, address, phone number, fax number and email address of the person to whom the formal contract proposal should be sent if an Associateship Program is approved.

Item No. 7. List specialized research fields rather than broad disciplines; for example, state "analytical chemistry" rather than "chemistry".

Item No. 8. Include date of approval by Institutional Review Board. The National Academies must have a copy of the guidelines and a formal statement that the parent organization monitors, controls and assumes full responsibility for all studies involving human subjects.

Item No. 9 Limit to 1000 words. Include mission or objectives, organizational structure, number of personnel (technical and support), geographic location, type of research facilities, etc.

Item No. 10 Limit to 500 words for each major unit. Include objectives, organizational structure, number of personnel (technical and support), geographic locations, specialized research facilities, computer and library facilities, research project areas, degree levels of technical staff, cooperative research programs with other organizations, fiscal support, etc.

Item No. 11 List recent (last 10 years) pertinent technical publications in the open, refereed literature. Research advisors must be active (or recently active) investigators who will be available for technical advice and guidance. Provide proposed advisors' professional achievements.
