Visa Questionnaire

The International Visitors Office has designed a questionnaire to collect information regarding visa-related issues from the scientific community. If your visa case has been delayed for more than 60 days and you are willing to share this information, please submit a questionnaire.

This information will be used by our office to:

  • report trends with visa applications;
  • generate reports on specific visa-related issues;
  • improve our ability to facilitate the free flow of scientists; and
  • inquire about the status of pending applications whenever possible

Once you have submitted a questionnaire, your case will be added to our database. Please submit only one questionnaire per applicant. If you have a correction to the information you submitted or a significant change in your situation (i.e. you received your visa, you canceled your travel plans, etc.) please send this information by email to visas@nas.edu. Include the applicant’s name, passport number and date of birth in the subject line.

If you have difficulty completing the questionnaire or suggestions for improvement, please email us at visas@nas.edu.

Note on administrative processing: If your application is pending administrative processing/security review (ex. Visa Mantis), please only submit a questionnaire if you have been waiting for at least 60 days after the date of your interview at the consulate.

DISCLAIMER: The purpose of this database is to collect information on visa issues to enable the National Academies to respond to problems related to visas. It is very important to note that the International Visitors Office (IVO) is not an office of the U.S. Department of State, which is in charge of all U.S. visa applications. The IVO is an office of the U.S. National Academy of Sciences (NAS) which is a non-governmental organization with no control over U.S. visa regulation and therefore cannot issue visas or control the process in any way. Our office is gathering information on the problems that visa applicants from the scientific community are experiencing with the process and we are reporting any cases that have been pending for longer than 60 days to the Department of State.

This information will only be accessible to staff of the International Visitors Office. No personal information, including an applicant’s name or email address, will be used for any other purposes. Your email address is required to contact you in the event that this office can assist in your case.
